On September 9, Dutch investigators released the first report on Malaysian Airlines Flight 17. The plane crashed in eastern Ukraine on July 17, killing all 298 passengers on-board.

✓ investigators rule out human error or mechanical failure as contributing factors in the MH17 crash;
